Your First Visit

Your first visit will typically be for an initial examination which will include an oral cancer screening, a restorative exam to check for decay, missing teeth and defective restorations, and a periodontal exam to check the condition of the supporting bone and gums.

Medical History

All patients must complete and update our "Patient Information Form". Please be sure to provide complete, up-to-date information including recent illnesses and hospitalizations and current medical conditions. If you have a medical condition requiring premedication prior to treatment, please let us know in advance so we may prescribe the appropriate drug for your appointment. We will also need the names, doses and frequency of any medications including over-the-counter and herbal remedies that you are taking. This information will help the doctors select the safest and most effective method of treatment for you.

Treatment of Minors

Minors (under 18 years of age) must be accompanied by a parent (or guardian) for their appointment.

Regular Appointments

Regular checkup appointments typically require 40 to 60 minutes, and patients are seen by appointment only. That appointment time is reserved specifically for you, and we make every effort to be on time. We ask that you extend the same courtesy to us and our patients by keeping that appointment. If an emergency situation should arise requiring that you change an appointment, please notify us as soon as possible. Our policy is to not charge for missed appointments with less than 48 hours advance notice. However, if three (3) appointments are missed by your family, and we have not received notice within 48 hours of those appointments, a charge may be assessed at the rate of a normal office visit and we reserve the right to dismiss the family from our practice.

Emergency Treatment

If you have a dental emergency during a weekday, we will make every effort to see you that day with minimal impact on our scheduled patients to alleviate your discomfort. In most cases, you will be reappointed for more definitive treatment.

Insurance

In most cases, we will process your insurance and as a courtesy we will file claims and handle insurance questions on your behalf. Also, we will attempt to plan your treatment in a manner that will take the most advantage of your coverage. Insurance is a contract between you and your insurance company while our sole responsibility is to provide you with professional care. We will not become involved in disputes between you and your insurance company regarding deductibles, co-payments, covered charges, secondary insurance, and usual & customary charges.

Payments

In an effort to keep fees down while providing you with a high level of professional care, we have established the following financial policy:

  • If you do not have insurance, payment in full is due at the time of service unless other arrangements are made in advance. We accept cash, checks and Visa/MasterCard.
  • If you have insurance, you are responsible for your estimated portion of the charges not covered by insurance at the time of service. Your estimated portion is based on a prior claims history with your insurance carrier or a preauthorization filed by us with your insurance carrier. Due to the large number of policies and the numerous changes annually, please realize that we can not guarantee 100% accuracy.
  • If your insurance company has not paid the full balance within 45 days of the service, you have 15 days to pay the balance. Interest charges at the rate of 1.5% per month (18% per annum) will be added to unpaid balances after 90 days from the date of service. If your insurance company pays more than the fee charged, we will send a refund check for the difference to them immediately. If you overpay your full balance, we will either issue to you a refund or carry a credit on your account according to your wishes.

Billing Statements

A computer generated billing statement will be sent every 30 days to all accounts with an outstanding balance over $3.00. Accounts with balances under $3.00 will receive a statement when the account balance increases or periodically if there has been no activity in the past 6 months. In some instances the statement may seem confusing, and we may provide a note of explanation. If you have any questions, please contact us.
